Job description: Responsibilities:

• Meet clients and families in their home setting
• Develop a plan of care including nursing and support services that meet the client’s needs
• Determine what care giver team members are appropriate to meet the client’s needs and manage that team ongoing
• Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.

Requirements:

• COVID vaccination required
• Licensed Registered Nurse. BSN preferred
• Supervisory experience
• Prior home care experience REQUIRED
• Private pay experience preferred
• 3+ years clinical experience
• Valid US Drivers License
