Job description: The purpose of this role is to coordinate and conduct assessments for new referrals (both adults and children) to determine their suitability for admission to the service.This involves:
- Evaluating needs through in-person and virtual (Teams) assessments in collaboration with the Multi-Disciplinary Team (MDT) and Operations.
- Making recommendations on service requirements, support needs, and staff training based on assessment findings.
- Ensuring smooth referral processing by completing documentation, coordinating assessment packs, and communicating outcomes (including drafting rejection letters when necessary).
- Representing the organization in referral meetings and assessments, ensuring alignment with policies and service capabilities.
Role Responsibilities: (Include, but not limited to)Manage and conduct comprehensive assessments for new referrals (both adults and children) to determine their suitability for admission to the service. This involves:
- Leading the assessment process by conducting virtual (Teams) and in-person evaluations in collaboration with the Multi-Disciplinary Team (MDT) and Operations.
- Providing expert recommendations on service suitability, care requirements, and necessary staff training to support potential admissions.
· Ensuring efficient referral coordination by completing documentation, preparing assessment packs, and liaising with Senior Business Manager, CEO Office for processing.
- Representing the organization professionally in referral meetings and assessments, ensuring decisions align with policies and service capabilities.
- Handling refusal communications by drafting and submitting rejection letters when the service cannot accommodate a referral.
The Post Holder will ensure that new admissions are thoroughly assessed, appropriately supported, and smoothly transitioned while maintaining compliance with organizational standards.Post Admission:
- Monitoring and reporting on new admissions for the first 12 weeks, providing weekly written progress updates at referral meetings (every Thursday)
Additional supports:
- Assessing the need for additional supports post-admission, completing reviews with the Director of Support (DOS), and presenting findings to the COO.
- Ensuring compliance with policies while representing the organization professionally in decision-making processes.
- Making evidence-based recommendations on care requirements, staffing, and training needs to support successful admissions.
- Coordinating referral documentation, including assessment packs and rejection letters, in liaison with Business Support (CEO Office).
Key Criteria Alignment:The Candidate will demonstrate the ability to lead the end-to-end assessment, admission, and post-admission monitoring process for individuals (adults and children) referred to the service, ensuring person-centred, compliant, and high-quality care.Clinical Expertise: NMBI/Coru-registered nurse/allied professional with 3+ years’ post-qualification experience, including 1 year in disability residential services (Adults Children)
- Regulatory Knowledge: Proven understanding of HIQA/TUSLA frameworks and risk assessment.
- Technical Skills: Advanced report writing, needs assessments, and care planning.
- Operational Requirements: Full driving licence for in-person assessments.
· Person Centred· Experience of completing individual assessments of needs, including personal planning & Risk assessments· Excellent report writing skills· Ability to critically analyse complex information· Key understanding of Irish regulatory and policy – HIQA, Tusla etcIdeal Candidate Profile:✔ Advanced Clinical Credentials:
- Essential: NMBI/Coru registration + 3 years’ experience.
- Desirable: QQI Level 8+ qualification + 5+ years’ experience, including supervisory roles.
✔ Specialised Expertise:
- Essential: Disability/residential experience + risk assessment skills.
- Desirable: Formal risk management/mitigation training.
✔ Leadership & Compliance:
- Proven ability to mentor teams and uphold HIQA/TUSLA frameworks in high-stakes decisions
