The IPAC Practitioners facilitate infection control activities and are change agents with the primary goal to support and build IPAC capacity in congregate living settings, reducing the risk of infections and hospital transfers .The IPAC practitioner directly reports to the IPAC Hub Manager and will work with the regional IPAC hub to provide expert guidance and sustainability planning in partnership with community congregate settings including LTC partners, retirement homes and others (e.g., group homes, supportive housing and shelters).Accountabilities:
- Provide clinical IPAC leadership to congregate living settings within the Central West region
- Oversee and facilitate local IPAC communities of practice
- Flag infection control concerns to congregate living settings and IPAC hub manager
- Support IPAC planning and capacity building
- Provide IPAC best practice and clinical direction
- Conduct clinical IPAC assessments and audits
- Provide rapid access to IPAC expertise 24/7
- Work in collaboration with Public Health, Ministry of Health and Ontario Health
- Report on assessments and risks for identified homes and provide IPAC education appropriate to a variety of settings
Qualifications
- A degree in health related discipline preferred (e.g. nursing, laboratory, epidemiology and other allied health)
- Current registration and in good standing with a regulatory body in Ontario
- Previous experience as an infection control practitioner in an acute care setting or long term care home setting is preferred
- Completion of an entry level 1 infection control course required
- Certified in Infection Prevention and Control preferred or commitment to complete within two years required
- Knowledge and/or job related experience in health care practices
- Strong knowledge of infectious diseases, microbiology, epidemiology and statistics
- Proficiency in the use of computer databases, spreadsheets, MediTech and literature search
- Demonstrated ability to work collaboratively in a multidisciplinary team environment
- Self-directed, well-organized and detail-oriented
- Excellent interpersonal, communication and presentation skills

- Excellent attendance, punctuality and work record
- Demonstrated skill in teaching and strong written communication skills
- Able to participate in the IPAC on-call rotation and travel within the central west region
