Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.About the roleTo oversee and manage the delivery of a high-quality Aged Care Assessments (including Home Support Assessments and Comprehensive Assessments) Service under the Single Assessment System within the VIC3 Service Area, ensuring compliance with relevant legislative, operational, and performance requirements. The role involves leading and collaborating with internal and external stakeholders, including subcontractors (Grampians Health and Bendigo Health), to achieve effective outcomes.This position is responsible for driving cross-agency collaboration, maintaining transparent communication, and fostering a culture of continuous quality improvement. Additionally, it includes strategic planning, risk management, and ensuring the provision of well-trained staff to deliver assessment services.
