Western Plains Correctional Centre (Western Plains) is Victoria’s newest maximum-security correctional facility, designed to house up to 1,248 people in custody.Western Plains is a multi-purpose facility that adapts to the evolving needs of Corrections Victoria. It features purpose-built modern amenities including medical wards, a cultural centre and vocational training facilities. It utilises cutting-edge technology to keep the facility secure, allowing staff to focus on supporting the rehabilitation of people in custody.Western Plains will open mid-2025 and Corrections Victoria are now recruiting key roles to help shape the future operations of the facility.About the roleThe Assessment and Transition Coordinator (ATC) is part of the Offender Services team which is responsible for the coordination and delivery of rehabilitation, reintegration and specialist support services at Western Plains.As the ATC, you will oversee the administration of the Corrections Victoria Reintegration Pathway, including the coordination and facilitation of
- risk and needs assessments of people in custody,
- transition and discharge planning,
- parole applications, and
- referrals to pre- and post-release services.
You will work directly with individuals in custody, guiding their transition into, through, and out of the prison system. This will also include facilitating sessions and services and providing practical assistance to Offender Services teams.This role involves collaborating with multi-disciplinary teams to ensure integrated case management, supporting contracted service providers, and contributing to a positive, pro-social environment that fosters rehabilitation and reintegration.About youYou understand the specific transition and reintegration needs of people in custody and can administer risk and welfare assessment tools. You communicate effectively and sensitively with diverse groups, including Aboriginal and Torres Strat Islander communities. You have experience working with individuals and groups who are difficult to engage.A relevant tertiary discipline is highly desirable, and experience in human services, welfare practice or related fields is highly regarded.How to applyClick the Apply button on this ad.
